Compassionate Care Alliance

This funding will provide operating support for Compassionate Care Alliance, founded by Hospice Foundation in 2000 as the End-of-Life Coalition.  The organization became CCA in 2002 as a separate non-profit and today offers a range of community education programming on planning and preparing for the end-of-life.  This grant will help CCA present workshops for the public on advance health care directives, and outreach to physicians and other groups on the legally mandated Physicians Order for Life-Sustaining Treatment (POLST) form.
